Covenant Health Senior Friendly Environmental Scan
Senior friendly care focuses on providing care environments, processes, supports and services that adapt to seniors’ physical and cognitive needs, promote good health, maximize safety and involve patients and their caregivers as partners in their care. It enables seniors to regain their health and transition smoothly from hospital to the level of care that best meets their needs.
Covenant Health compiled an inventory of senior friendly programs, services, initiatives and research-based projects to inform its action plan on ‘care that honours seniors’ and to provide an organization-wide assessment of senior friendly environments across our centres. The scan involved 19 Covenant Health acute and long term care centres in urban and rural settings.
The Network worked in partnership with site administrators and staff in Covenant’s rural and urban acute and long term care centers.
The environmental scan will support the future planning and prioritization of senior friendly initiatives and collaborations across Covenant Health. A meeting of the 19 contributing sites in October 2016 began identification of next steps and areas of priority for the coming one to three years required to realize milestones in Covenant’s Care that Honours Seniors Strategy.
